Below is a list of useful documents and forms in PDF format that can be downloaded or opened online to view and/or print.

Instructions for viewing or downloading documents:

1. To Open/View the document - click on the document name.

2. To download to your computer - right click on the document name and click "Save Target As..."  Be sure to remember where you put it on your computer so you can open/view it once downloaded.

First Visit?

The first time you come to Homes on the Hill CDC for a class or an individual appointment, you will need to bring the following documents filled out to the best of your ability: 

• Intake Package - forms for your first visit to Homes on the Hill

• Monthly Budget – monthly spending plan

• Supporting Documents Checklist - review this list and bring all the listed documents that relate to you. These documents support your case and explain your situation to the HOTH counselor.
